Coaches Reimbursement - This form is for Coaches receiving their 1st license as required by the YAYSL to coach in our league. The YAYSL will reimburse up to $75 of the license fee, after completing the season. Submit this form to your Communities Representative for the YAYSL, so they can be submit it to the League President for Approval. After it is approved the Treasurer will mail out a check.

Host Site Reimbursement - This form is to be submitted by each Community Representative at the end of the season. The forms should be sent to the League Registrar

Fall Classic Registration Form - This form is to be submitted by each Community Representative no later than the September meeting along with a check for the tournament fees. The forms should be sent to the League Registrar
Fall Classic Tournament Team Declaration Form

Coaches List - This list is to be filled out in excel and emailed to the league Registrar by the June 30th. A copy of this form is also to be submitted along with the host site declaration form.

Game Day Report - This report is to be filled out and given to the Referee prior to the start of each game. The Referee is to fill out the sheet at the end of the game and give it back to the Coach. The Coach needs to report the score and any cards issued to their designated Score Keeper so it can be entered into the website. The Coach also needs to keep the completed form in case there are deputes, at which point a copy of the report will need to be sent to the League President for review.

Player from Another YAYSL Community Approval Form - This form is to be submitted by each Community Representative no later than the June meeting. This form is to declare any players who you want to roster but live in another community with teams entered in the YAYSL.

Host Site Declaration - This form is to be submitted by each Community Representative no later than the July meeting along with a check for the Team Registration Fees. The forms needs submitted with the League Registrar.
